Passports

The Henry County Library is a passport acceptance agency and is able to accept passport applications on behalf of the U.S. Department of State. U.S. citizens planning international travel may set-up an appointment to apply for their passports at the following location:

Patrons must set-up an appointment in advance to obtain a passport. Walk-ins will not be accepted! To book your appointment call 660-885-2612 or e-mail hcl@henrycolib.org.

Passport acceptance hours are:

Mon, Weds: 8:00 AM-8:00 PM

Tues, Thurs, Fri: 8:00 AM- 3:00 PM

 

Please note that passports usually arrive within 4-6 weeks of the application being processed. Expedited service for a 2-3 week arrival is available for a fee (see below for fee information). Anyone needing to travel sooner than 2 weeks should make an appointment at a passport agency or center. Go to travel.state.gov for more information.

 

When coming to your passport appointment please bring the following items (The Library reserves the right to cancel appointments if patrons do not bring the correct forms or information):

ADULTS: Patrons 16 years of age or older

DS-11 Form completed (do not fill out page one section 2) in black pen. See link below for a PDF of the DS-11 form.

Proof of U.S. citizenship and a photocopy of the front (and back if there is printed information). Proof of U.S. citizenship can be a CERTIFIED birth certificate (photocopies only will not be accepted), previous passport book or passport card, certificate of naturalization, certificate of citizenship (for other options please go to travel.state.gov).

Proof of Identity: You must present your original identification and submit a photocopy of the front and back. Proof of identity can be a valid driver's license, military identification, state, federal, or municipal government ID card. For other examples of acceptable forms of proof of identity please go to travel.state.gov). 

Passport photograph: Recent color photograph, full front view of face, 2 x 2 inches, meeting all passport requirements (for more information on acceptable passport photos please go to travel.state.gov).

For applicants whose current name is different than their Proof of Citizenship or Proof of Identity due to marriage, divorce, court ordered name change, or other reasons please provide Certified or Original copy of marriage certificate, Court order of name change, divorce decree, adoption decree (photocopies will not be accepted).

Passport and Execution Fees: A check, money order or bank draft made out to the 'U.S. Department of State' for each individual passport being requested (cash and credit cards will not be accepted). For current fees please see below. A $35.00 execution fee per passport to the Henry County Library is also required. The execution fee can be paid via check, cash, or credit card and fees for multiple passports can be paid in one lump sum. 

 

ADULT Applicant Fees:

Adult Passport Book: $110.00                    Adult Passport Card: $30.00                        Adult Passport Book and Card: $140.00

Expedite Fee: $60.00                                Overnight Delivery Fee: $15.89

 

MINORS: Patrons under 16 years of age

IMPORTANT: All minors, regardless of age, must appear in person when applying for a passport!

Both birth parents or the child's legal guardian(s) must appear and present the following (if one parent is unavailable please see below):

- DS-11 form completed in black pen (do not fill out page 1 section 2). A PDF of the DS-11 form can be found at the bottom of this page.

- Proof of the child's U.S. citizenship and a photocopy of the front (and back if there is printed information). Proof of US citizenship can be a CERTIFIED birth certificate (photocopies only will not be accepted), previous passport book or passport card, certificate of naturalization, certificate of citizenship (for other options please go to travel.state.gov).

- Evidence of the child's relationship to the parents/guardian(s): Certified U.S. birth certificate with parent's names, Certified adoption decree with adopting parents' name, Certified court order establishing custody or guardianship.

- Original parental/guardian issued identification and a photocopy of the front and back side: Identification can be a valid driver's license, military identification, state, federal, or municipal government ID card. IF ONLY ONE PARENT APPEARS YOU MUST ALSO SUBMIT THE FOLLOWING:

- Second parent's notarized written statement or DS-3053 (including the child's full name and date of birth) consenting to the passport issuance for the child. The notarized statement cannot be more than three months old and MUST be signed and notarized on the same day and MUST come with a photocopy of the front and back side of the second parent's government issued photo identification OR

-Second parent's death certificate if second parent is deceased OR

-Primary evidence of sole authority to apply such as a certified court order OR

-A written statement or DS-5525 (made under penalty of perjury) explaining in detail the second person's unavailability.

 

THE LIBRARY RESERVES THE RIGHT TO CANCEL PASSPORT APPOINTMENTS IF INDIVIDUALS DO NOT BRING ALL REQUIRED DOCUMENTS!
